Kindergarten Health Reference Sheet

Everything stated below is per Utah Law

Immunizations

  1. Immunizations are state law. 
  2. Students must have all the required immunizations completed prior to the first day of kindergarten. 
  3. Refer to the Immunization Requirements flyer in this packet. 
  4. Immunization Records
  5. All immunization records must have a day, month and year noted with an official signature or clinic name. 
  6. Immunization records brought in by parents must be an official document or it is not acceptable. 
  7. Doctors can fax immunization records to 801-826-9176 or email them to angela.jackman@canyonsdistrict.org. 
  8. Conditional/Non-Compliance
  9. A student is considered on conditional status if they are on schedule for immunizations.
  10. If any student on conditional status is 30 days past due for any required immunization, they are out of compliance and will be excluded from school until they receive the immunizations or claim an exemption. 
  11. Students will not be admitted to school if they are past due for any immunization. 
  12. Exemptions 
  13. Medical exemption forms can be obtained from doctor
  14. Religious or personal exemption forms can obtained from local health department 

 

ALL OF THE ABOVE MUST BE COMPLETED PRIOR TO THE FIRST DAY OF KINDERGARTEN OR THE STUDENT WILL BE EXCLUDED. 

Health Concerns 

  1. If the student has a health concern that needs attention at school, the parent(s) must contact the school and complete a “Request for Health Care Services” form. 
  2. This can be found on the Canyons District or Quail Hollow websites. 
  3. If a Health Plan is needed, the parent(s) must contact the school each year it is to be put in place.

 

 Medication Forms 

  1. No medication or treatments (prescription or over the counter) can be given at school until a doctor has signed the medication form. 
  2. All medications must be in the original container (prescription or over the counter), brought in by the parent(s) and counted with office staff.
  3. Students cannot carry any medications at school EXCEPT for asthma inhalers that prescribing doctors indicate on the medication form that the student may carry and self administer the medication.
